Everyone entering the health care system is entitled to receive qualified professional assistance in planning for necessary ongoing health related needs at the time they leave the system.
The Indiana Continuity of Care Association exists to enhance the concept of continuity of care as both an essential part of the health care continuum and as an integral part to total patient care.
We serve as an advocate for legislation and a voice for community related health concerns and issues by identifying and communicating them to members. Working in tandem with other allied organizations, Indiana Continuity of Care Association supports standards of professional competence which identify, structure, and promote a multi-disciplinary approach to continuity of care.
